Traumatic Brain Injury Waiver

Program Information


The Home and Community Based Services Medicaid Waiver for individuals with Traumatic Brain Injury (HCBS/TBI) is a community-based program, specifically designed to assist adults who have sustained a traumatic brain injury. The intent of this waiver is to provide specialized services to the individual in order to terminate and/or circumvent a skilled nursing home or rehabilitation center admission. All services through this Waiver are provided under the individual's NYS Medicaid. 

ELIGIBILITY
  • Be enrolled in NYS Medicaid
  • Be between ages of 18 and 64 with a physical disability or be age 65 and older upon application to the waiver
  • Have a diagnosis of traumatic brain injury or related diagnosis
  • Be assessed to be in need of a nursing facility level of care
  • Choose to participate in services through the TBI Waiver

SERVICE COORDINATION (SC)

Service Coordination is a person-centered program provided by qualified personnel for individuals served. Our highly educated and experienced Service Coordinators assist consumer in exploring and securing supports and services that best meet consumers' needs and desires while allowing consumers to live as independently and productively as possible. In addition, Service Coordinators assist in the development, implementation, and monitoring consumer's Service Plan (SP). 

The Service Coordinator works in partnership with consumer and acts as an "advocate" and a "service broker" guiding consumers in negotiating the complex health care and service system. Service Coordinators also assist consumers in improving their functional independence by identifying and accessing community resources (e.g., finances, home care, transportation, vocational, mental health and other community based services) that can meet their environmental, personal, or interpersonal needs. 

The Service Coordinators at Halo coordinate assessments, make referrals, provide information regarding benefits and entitlements, monitor, and maintain most services/supports as they apply to the consumers.

COMMUNITY INTEGRATION COUNSELING (CIC)

The challenges faced by an individual with Traumatic Brain Injury (TBI) and their family are life altering. Halo Network recognizes the need to support individuals and their families to successfully navigate all the emotional roadblocks and changes in their everyday lives. Our qualified and caring staff will assist the person with TBI and the family in setting the path toward integrating back into their community to the fullest extent possible.

COMMUNITY TRANSITION SERVICES (CTS)

Community Transition Service provides funding for the reasonable costs of one-time set-up expenses for individuals transitioning from a nursing home to their own home or apartment in the community. Reasonable costs are defined as necessary expenses for program participants to establish a desirable living space.

INDEPENDENT LIVING SKILLS TRAINING (ILST)

This service is designed to provide additional support for NHTD in their homes and communities. ILST services are individualized to meet each participant's needs. ILST plans are individually established to improve and maintain the ability of participants to live as independently as possible in the community. 

Halo Network's ILST services include skill development training, which includes community inclusion, transportation, problem solving, money management, pre-vocational and other life skills required for successful community living. 

Our experienced and dedicated specialists work with participants in achieving valued outcomes including participants' dreams, goals and desires. These outcomes allow participants to become independent, participate in community activities, and develop new skills.

STRUCTURED DAY PROGRAM

Halo Network provides a structured, comprehensive program that provides functionally impaired individuals with socialization, personal care, nutrition, supervision, and monitoring in a protective setting. The program also provides other services and support, such as transportation, caregiver, information, and assistance. In addition to addressing the individual's needs, Halo Network’s day program focuses on maintaining the participant's skills and ability to live as independently as possible in the community. The services provided include assessment, training and supervision to an individual with self-care, task completion, communication skills, interpersonal skills, problem-solving skills, socialization, sensory/motor skills, mobility, community transportation skills, reduction/elimination of maladaptive behaviors, money management skills, and skills to maintain a household. 

Social, recreational, therapeutic and educational activities are offered throughout the day, including exercise classes, arts & crafts, discussion groups, and music. Group activities are designed to meet the individual needs and abilities of each member. Senior adult day care services are provided in an attractive, home-like setting. Door-to-door transportation is available. Nutritious meals and snacks are served.

ENVIRONMENTAL MODIFCATION SERVICES

Environmental Modifications (E-mods) are internal and external physical adaptations to the home, which are necessary to ensure the health, welfare and safety of the waiver participant. These modifications enable the waiver participant to function with greater independence and prevent institutionalization.

Halo Network, Inc provide Home modifications, adaptations and repairs to ensure participants safety in their own home. The goal of the service is to provide participants with a greater level of independence allowing them to live at home with increased accessibility and comfort.

E-mods in the home include the purchase and/or installation of:

  • Ramps
  • Lifts: hydraulic, manual or electric, for porch, bathroom or stairs (Lifts may also be rented if it is determined that this is more cost-effective.)
  • Widened doorways and hallways
  • Hand rails and grab bars
  • Automatic or manual door openers and doorbells
Bathroom and kitchen modifications, additions or adjustments to allow accessibility or improved functioning, include:
  • Roll-in showers
  • Sinks and tubs
  • Water faucet controls
  • Plumbing adaptations to allow for cutouts, toilet/sink adaptations
  • Turnaround space changes/adaptations
  • Worktables/work surface adaptations
  • Cabinet and shelving adaptations
